Just to check; the default maximum size limit for Postfix/Sendmail is often 
~20MB.  Have you made certain that your MTA (and everything after) are 
configured to accept messages of this size (IE, more than 25MB)?  You might 
find that on edge cases, the message arrives, but cannot be distributed on 
due to its increased size from new headers.

If you are getting a slow query logged on an insert, this might indicate 
problems with the indices, or with the underlying disk
