So, we've gone through a couple of rounds with the DBAs increasing the

in MySQL for Sympa? I know this is going to be related to the largest
size of a message. Still, I would have thought that 128mb would have
been plenty.

FWIW we've got it set at 100MB, after several similar rounds of raising it and seeing that error and raising it again - and what we eventually realized was that it was the digests that were always pushing it much higher than we expected. Ie. even if your max message size is set to something reasonable like 10MB or 20MB, a handful of those going out in one digest quickly adds up to 100MB or more.

So that's something to keep in mind when setting it. If you've got people sending a lot of large files, and a lot of people using MIME digest mode, it wouldn't surprise me if you needed a max packet size of 200MB or 300MB, or more. (At some point I imagine you might run into some kind of limit on how high you can set this limit, but I have no idea.)
