The way I wrote the patch, they need to be applied per-list.

Originally I wrote them as global settings, but I felt that - particularly
for a unofficial patch - they should default to normal behaviour, and be 'opt
in' on a per-list basis. You'd only need to apply them to lists where you
expect external users, which for us is a minority of our full set, though for
some users it may be all lists...

The defaults for the settings are set in the patch, but it could be altered
to make them seek out global settings instead. I have an updated version I'm
working on that adds more options for when to apply the changes to the From
header; I'll see about adding a global default setting as well.

If you have a lot of lists and want to apply globally, then you could write a
simple script to append to the list config file and call sympa.pl to recreate
the .bin files?
