You did not mention what Sympa version you are using ; I presume 3.4.4.3.
Here are a few things you should check/try :
  1. check if the message was not previously confirmed
  2. are there subscribers in the list (try a review)
  3. set sympa to a higher debug level (log_level to 2 or 3) and have another try with the CONFIRM
